INDIANA’S STATE TRANSITION TEAM VISION STATEMENT: Our Vision is to assist in creating a comprehensive community-wide system ensuring positive and effective transition experiences for all children and their families. MISSION STATEMENT: The purpose of Indiana’s State Transition Team is to assist in developing, coordinating, and implementing activities which enhance and facilitate communities’ abilities to provide an effective, seamless transition process from once service provider/environment to another for children birth to third grade.
